Medication Management & Education

Medication setup and administration (including injections as ordered), reconciliation after hospital/clinic visits, side-effect monitoring, and teaching for safe self-use.

Wound Care & Post-Operative Care

Wound assessment, cleansing, and dressing changes; pressure-injury prevention; monitoring for infection; coordination with wound-care specialists when needed.

Chronic Condition Monitoring

Ongoing assessment of vitals and symptoms for conditions such as diabetes, heart failure, COPD, and hypertension; early identification of changes and timely escalation to the provider.

Catheter, Ostomy & Tube Care

Maintenance and teaching for urinary catheters, ostomies, and enteral feeding tubes per the care plan.

IV & Injectable Therapies (as ordered)

Administration and monitoring of IV antibiotics, hydration, and prescribed injections with attention to safety and infection control.

Patient & Family Education

Practical coaching on disease management, equipment use, diet and activity guidelines, and infection-prevention best practices.

Care Coordination & Reporting

Regular communication with physicians and the broader care team, timely documentation, and updates to the plan of care as needs evolve.
